Position Overview:

The Case Manager will work with a multidisciplinary team to provide individualized person centered services to adults with a severe mental illness. They will provide individualized strengths based case management and skills training services consistent with the individual’s treatment plan with a specific focus on skills acquisition, building and repairing natural supports, and community integration. They will work with social service agencies, community partners, naturally occurring resources, and stakeholders to ensure that individual recovery goals are met.

 

Required Skills:

  • Experience with the Recovery model, Intensive Case Management, Motivational Interviewing, Trauma-informed care and Integrated dual- diagnosis treatment a plus
  • Ability to respond appropriately to crisis situation.
  • Good organizational and computer skills; previous experience with electronic medical records preferred.
  • Effective time management skills and ability multitask
  • Knowledge and/or experience with community based rehabilitative case management services within a mental health management care system.
  • Knowledge and comfort with psychopharmacology terminology.
  • Maintain current Oregon or Washington Driver’s License and vehicle insurance as required by state law.
  • Ability to pass a criminal history check.

Physical Requirements:

  • Use hands to manipulate, handle, feel, and control items or equipment

Education:

  • Must meet qualifications for a Qualified Mental Health Associate (QMHA).
  • Bachelor’s degree in human/social services field (social work, public or community health, psychology) or related filed, or 3 years of experience working in human services.

Job Type: Full-time

Pay: $48,672.00 - $52,540.80 per year

  • 8 hour shift
